Insurance coverage can vary by plan and network. We recommend contacting your health plan to verify in-network coverage prior to your visit.
If you have an HMO plan, Page Family Practice must be listed as your Primary Care Provider (PCP) before your appointment.
Copays, coinsurance, deductibles, and outstanding balances are due at the time of service.
Cash-pay options are available for patients without insurance coverage.
Most secondary insurance plans are accepted when paired with Medicare.
Appointments can be scheduled by phone or through the Patient Portal. We kindly ask that patients provide at least 24 hours’ notice for cancellations whenever possible. Same-day cancellations and late cancellations may be subject to a $25 fee, and missed appointments may result in a $25 no-show fee. Patients arriving late may need to be rescheduled depending on provider availability. Same-day visits and telehealth appointments may be available for select concerns.
Please note that some services offered by our practice may not be covered by health insurance and may be considered self-pay. Coverage varies by plan. While our team makes every effort to inform patients when services may not be covered, patients are ultimately responsible for understanding their individual benefits.
